Count on UNC Health Nash to keep your medical records safe and confidential. Work with our health information management staff to request a copy of your records or release them to an authorized provider.
Your Online Medical Record
Sign up for a free My UNC Chart account to access your electronic medical record for visits and services that occur on or after September 22, 2018. You’ll see details about the care you receive from UNC Nash and other UNC Health Care providers. My UNC Chart is a secure online “patient portal” that allows you to manage appointments, get select test results, see doctor visit summaries, and view hospital admissions.
Please contact the Medical Records Department at Nash (HIM) for availability of archived records or records before September 22, 2018.
How to Request Your Medical Records
Fill out an Authorization for Disclosure of Health Information [PDF] form to get a complete copy of your Nash UNC medical record or authorize us to send your record to a provider for follow-up care.
Complete, sign, and return your form to UNC Nash by:

- In-person drop off – Check in at the Nash General Hospital main entrance for directions to our office.
Send Your Medical Records to a Provider
The provider on your authorization form may receive your records for follow-up care. We can fax your records the same day we get your request at no charge. We don’t fax medical records for non-patient care purposes.
Ways to Receive Your Medical Records
When you ask for a copy of your medical records for personal use, copying fees will apply. You’ll get a bill in the mail from ScanSTAT Copy Service with your medical records.
You have the option to receive your medical records by:
- Mail – Processed within three to five business days of receipt of your completed form (fees apply)
- In-person pick up – Get your records at our health information management department once we notify you they are ready for pick up
How to Update Your Medical Records
Request a change to your protected health information (PHI) with the Medical Records Amendment Form [PDF]. Complete and submit your form to the health information management department by:

UNC Health Nash must act on your request for an amendment no later than 60 days after receipt of your request. If UNC Health Nash is unable to act on the amendment within the required 60-day time limit, UNC Health Nash may extend the time for such action by no more than 30 days, provided that:
- A UNC Nash Privacy Officer provides you with a written statement of the reasons for the delay and the date by which action on the request will be completed; and
- UNC Health Nash may have only one such extension of time for action on a request for an amendment.
You’ll receive a written notice of acceptance or denial of your amendment request.
