My Nash Chart is our previous patient portal that can still be used to access your electronic medical records at Nash UNC for visits and services that occurred before September 22, 2018. For visits and services that occur on or after September 22, 2018, use My UNC Chart.
My Nash Chart is a secure online program designed to give our patients convenient 24/7 access to their Personal Health Record. It is a free service offered by the hospital as a courtesy to our patients and its use is optional.
The records we create while you’re a patient at Nash UNC Health Care can be accessed through the My Nash Chart on any device (SmartPhone, tablet, or personal computer) with an internet connection. These are records generated only while being a patient in recent years at Nash UNC Health Care. Your private physician records are not included.

What is the My Nash Chart Patient Portal?
The My Nash Chart Patient Portal is a convenient and secure health management tool that you can use anywhere you have access to the Internet. Through the portal, you can:

Is there a cost for My Nash Chart?
My Nash Chart is a free service provided to you by Nash UNC Health Care.
How do I sign up for My Nash Chart?
During your patient registration, we will ask you to provide your email address. Within a few days, you will receive an e-mail invitation to create your account. Once you receive your e-mail invitation, click on the link and follow the instructions to set up your My Nash Chart account. Make note of your username and password and keep it in a safe place.
How can I access My Nash Chart?
- For future visits to My Nash Chart after you have completed the setup process, you can log in via the “My Nash Chart” link through Nash UNC Health Care’s website, www.nashunchealthcare.org.
- Remember, use this link www.nashunchealthcare.org after you have received a portal invite through email and completed the sign up process.
How soon after I register will my health information be there?
Your visit information should be available immediately.
What is needed to access the patient portal?
All you need is a computer or device with internet access and a valid e-mail address. A printer is necessary if you want to print your electronic health record.
Can patients sign up without being seen?
At this time, there is not an option to self-enroll. Patients will be offered the option to provide their email address during the Registration process in certain hospital locations initially, with future expansion to all locations. An invitation to the portal will be generated to the patient through email.

What is included in the My Nash Chart health record?
The My Nash Chart Patient Portal includes a view of clinical data from our Nash UNC Health Care medical record. Your health record includes:
- Clinical Record Summary
- Clinical and Lab Results Radiology Reports
- Pathology Reports
- Discharge Documents
- Education Documents
What lab results can I view in My Nash Chart?
You are able to view all of your lab results except HIV and Genetics results.
How do I view my health information?
- The “Health Record” section includes different categories of information.
- The “My Health Profile” section contains information about your medications, allergies, immunizations, health issues and past procedures.
- The “My Lab Results” section will have select test results.
- The “My Documents” section contains summaries of hospital visits, as well as any educational materials that were provided to you as part of your visit.
- The “My Radiology Reports” section will have your radiology or imaging services reports.
- The “My Pathology Reports” section will include pathology reports. The “My Procedures” section will include previous history of procedures that were done.
- The “My Visit Summaries” section allows you to view or download a visit summary or “Continuity of Care Document” (CCD).
What is a "Continuity of Care Document?"
A Continuity of Care Document (CCD) provides a snapshot in time of specific medical information that can be viewed by you, downloaded and saved for future use or transmitted to your physician for follow-up care. The transmission of personal health information is performed using Direct-trusted messaging. Direct-trust messaging is an easy and secure clinical messaging system within the patient portal that allows patients to send clinical information to their primary care physician office for follow-up care. A Direct-trusted e-mail address would need to be obtained from the office in which you are sending. Please contact your physician office for additional information.
My Nash Chart for my Family
Can I access my child's or parent's health information?
Yes. Proxy access to My Nash Chart can be granted to parents of children under age 13, as well as caregivers of elderly patients. For minor patients over 13, visit the Health Information Management Department with a valid photo ID to complete the invitation process. For patients with Guardianship or Health Care Power of Attorney documents, visit the Health Information Management Department with the appropriate documentation and a valid photo ID for My Nash Chart set up.
Who else will have access to my information?
Only you and those whom you grant proxy access will have access to your information.

Can I use My Nash Chart for emergencies?
No. Always call 9-1-1 in an emergency.
Can I pay my bill?
Bill pay is not currently available in the portal. To pay your bill, you may call or come into the Customer Service department. Customer Service is available to assist you Monday-Friday from 8a.m. to 5:30p.m. You may contact them at (252) 962-8030 or (252) 962-8050.
Can I schedule or change an appointment?
Yes, you may request an appointment using the appointments tab. Fill out the form provided and we’ll review your request and respond within three business days.
Can I message my physician?
At this time, we are not offering messaging to physicians.
I forgot my password. What should I do?
Click the “Forgot password” link on the sign-in page to reset your password online. You may also call 1-877-621-8014 to request a new, secure password.
What if I have questions or trouble accessing my account?
For problems logging into your My Nash Chart account, call 1-877-621-8014. Technicians will be available to assist you.
I was logged out of My Nash Chart account, what happened?
We aim to protect your privacy and security of your information. While logged into My Nash Chart, if the webpage remains idle for 15 minutes or more, you will be automatically logged out of My Nash Chart. You will receive a time-out warning prior to being automatically logged out to give you the opportunity to extend your session time n My Nash Chart. We recommend that you log out of your My Nash Chart session if you need to leave your computer for even a short period of time.
Why do I see a Warning message at the top of the Portal?
Nash UNC Health Care has contracted with Cerner Corporation (Cerner) to provide a patient portal known as My Nash Chart. My Nash Chart displays best with the current version of an internet browser, plus 2 versions back. The currently supported browsers are Internet Explorer 9 - 11, Firefox, Safari, and Google Chrome. If you are using an older version of a browser (particularly common with Internet Explorer), you are receiving the warning message to assist with understanding that an upgraded internet browser version is available and could result in a better end user experience for the portal.
Is my Nash Chart secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through a secure invitation process, personal IDs, and password. Each person controls their password and the My Nash Chart account cannot be accessed without a user id and password.
Nash UNC Health Care has contracted with Cerner Corporation (Cerner) to provide a patient portal known as My Nash Chart. My Nash Chart is a web-based, secure portal that is hosted, maintained and operated by Cerner. My Nash Chart is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by My Nash Chart.
Is there an app for that? (Is there a My Nash Chart mobile application, and if so, for what platforms?)
My Nash Chart is based on reactive web design technology. It automatically scales to adapt to various screen sizes, including mobile device screen sizes. No mobile application is needed, because it works on all web-enabled mobile platforms.
How can I obtain a paper copy of my entire medical record?
Please visit the Health Information Management Department or contact them at (252) 962-8130, Monday-Friday, 8:30am-4:30pm.
What should I do if I change my e-mail address?
Please visit the Health Information Management Department, Monday-Friday, 8:30am-4:30pm to update your email address. Then, we will send a new invitation to your new e-mail address. To ensure the privacy of your medical information, we cannot accept a change of e-mail address over the phone. Please be sure to promptly let us know your new e-mail address, to ensure that you do not miss any notifications.
How can I delete My Nash Chart account?
Should you decide that you no longer wish to use My Nash Chart, you may delete your connection to My Nash Chart from within your account. This removes your access to My Nash Chart by removing Cerner's ability to use your My Nash Chart identity (user name and password) for sign-in. Deleting the connection does not notify Nash UNC Health Care that you no longer wish to receive communications via My Nash Chart. You must contact Nash UNC Health Care at (252) 962-8130 to notify us that you no longer wish to receive communications via My Nash Chart.
